PMAR
A Personal Medical Attendant Report (PMAR) encompasses a comprehensive reproduction of the client’s medical file. In the process, we kindly request the client’s General Practitioner (GP) to furnish historical medical data retrieved from their own files. This dataset may encompass reports from specialists to whom they have previously referred our mutual client for specialized advice and care.
BMR
A Brief Medical Report (BMR) is solicited when precise details about a previous illness or injury are needed. Typically, your client’s General Practitioner (GP) can furnish the necessary historical information for this report, either sourced directly from their own records or from reports received and archived by specialist medical practitioners.
FILE COPY
A file copy denotes the entirety of a client’s medical record, securely maintained by their General Practitioner (GP) or medical clinic. These medical records encompass detailed notes pertaining to client consultations and any other interactions with the practice.
QUESTIONNAIRES
To gather more in-depth information pertinent to their present health status, tailored questionnaires may be sought from the treating practitioner. These specialized questionnaires may cover areas such as mental health, joint and musculoskeletal impairment, diabetes, etc.
